This waiver applies to all Madcats Baseball games, lessons, training sessions, camps, and events.

Please read this waiver carefully before participating.

I, the undersigned parent or legal guardian, acknowledge and agree that participation in Madcats Baseball lessons, training sessions, camps, clinics, and related activities (“Activities”) involves inherent risks, including but not limited to physical injury, illness, or property damage.

Assumption of Risk

I understand that baseball and athletic training activities involve inherent risks such as falls, contact with equipment, contact with other participants, weather conditions, and other risks that cannot be eliminated. I voluntarily assume all risks associated with participation in the Activities, whether known or unknown.

Release and Waiver of Liability

In consideration for being allowed to participate, I hereby release, waive, and discharge Madcats Baseball, its directors, coaches, instructors, staff, volunteers, affiliates, and representatives from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any injury, illness, loss, or damage sustained during participation in the Activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Minor Participant Acknowledgment

I certify that I am the parent or legal guardian of the minor participant and that I have the legal authority to execute this waiver on their behalf. I consent to the minor’s participation and agree to all terms of this waiver on their behalf.

Medical Authorization

I authorize Madcats Baseball and its representatives to obtain emergency medical treatment for the participant if necessary. I understand that I am responsible for any medical expenses incurred as a result of participation.

Photo and Video Release

I grant permission for Madcats Baseball to photograph or record the participant during Activities and to use such images or recordings for promotional, instructional, or marketing purposes without compensation.

Acknowledgment and Agreement

I acknowledge that I have read and understand this Waiver and Release of Liability in its entirety. I understand that by signing, I am giving up certain legal rights. I voluntarily agree to be bound by its terms.
